How to: Add a check box to a form.
Solution:
In the Form Design view, select the 'Check Box' from the Toolbox, and select the location for the check box on the form.
1) If the Database window is not active, activate the Database window.
2) Click the 'Forms' tab in the Database window.

3) Select the desired form from the list box. (The selected form appears highlighted.)
4) Click 'Design'. (The selected form appears in the Design view and the floating Toolbox appears.)
5) Click 'Check Box' from the Toolbox. (The check box button appears depressed.)
Select the Check Box button.
6) Position the mouse cursor on the form area to select a location for the check box. (The cursor becomes a check box.)
7) Click location for the check box on the form. (Both the check box and the attached label box appear.)

8) (Optional) To set the properties of the check box or label box, do the following:
9) To close the property sheet, click the 'X' in the upper-right hand corner of the property sheet.
Click X button to close property sheet
NOTE: The properties will be saved only when the design view is saved.
